Full EAP paperwork concierge, billed to the clinic. Includes Letter of Support, Position Description, Training Plan, SPPA-30, Section G, HW019.

EAP Onboarding — doctor pays

Standard rate (incl GST)

$1,100

Same scope as the clinic-paid package, billed to the doctor.

Add-ons

· Supervisor Concierge add-on · $1,100 (incl GST). Auto-offered to the clinic when the supervisor misses their first reminder.
· Admin intervention · $55/hr (min $165). Triggered after 3 reminders to a party.
